Reporting survey rewards on tax return
    Hi,
I am on a h1 visa, but got about $1200 assurvey incentives, last year. They sent me a 1099 form, and reported it as incine. My questions: 1) do I have to report this as self-employed income? 2) does it impact my h1 status to be getting survey rewards? 3) how should I report this in my tax return??

When received 1099 form, you MUST report this amount on your tax return. This does not jeopardize your visa status.
