    Whether an affadivit issued by a notary is suffic ient to obtaina passport for change
    Whether an affidavit issued by a notary is sufficient to obtain a passport for change in name.

    If your name is changed by reason of marriage, all you need is your marriage certificate to make a change on your passport, or carry your marriage cert with you, in your passport until renewal comes up.

    You don't say where you live, from my answer, assuming in the US or Canada, or UK.

    A notary does not "issue" an affidavit. All they do is witness the signature. You can obtain a new passport in a new name with proof of your legal name change.
