    You need to find out who the 401(k) plan administrator is - typically a bank, insurance company, or investment company such as Vanguard or Fidelity - and contact them. If you have an old quarterly statement their contact information should be provided on it. If you don't have any old statements and have neglected to advise the company of your new mailing address I suggest that you call the HR department of your old employer and ask who you should contact.
