    I am the maternal grandmother whom has had custody of child since birth
    My grandchild carries her fathers last name I want to add mothers last name before child starts school. Mother and father have lost custody and only sees child because I don't want to keep them out of her life

    What type of custody do you currently have? That is going to determine what steps are going to be needed. Was this custody court approved ?
