    There are many types of liens: tax liens, mechanics liens, mortgage liens, judgments. Each has its own proper format.

    Just a tip. When posting a question here it works to your advantage to explain the whole situation rather than ask a brief question. Context can often mean the difference between a helfpul or unhelpful answer. The more we know about your issue the better we can advise.

    So, instead of just asking what the proper format is, explain why you need to apply a lien.
