Guardianship

One of my daughters want's to take guardianship of her nephew, Her nephew has been have issues and his grades is not good, having issues with my oldest daughter. My grandson has attention deficit disorder. Takes medication for this. So, he has lot of problems with his mother.

My oldest daughter does not mine turning over the responsibilities to my other daughter. What document do I need to have this drawn up.

I would appreciate any suggestions.
