    A QDRO (Qualified Domestic Relations Order) occurs when a former spouse or other becomes entitled to a share of your pension through a divorce settlement. You should be able to submit the proper forms yourself to the pension administrator.

    Thanks now do you know where I can get the forms to print so that I can do this QDRO
    I want to print out the form to do the QDRO but I cannot find a site with a blank form for me to fill in with my information do you know one?

    What State?

    This is a tricky process. You are far better off with an Attorney (and you may already know that).

    PLEASE stop posting the same question - and giving me a "disagree" when I can't answer.

    As I said right from the beginning I need to know what STATE you are in. Each STATE has a different form.

    I provided you with the basic information, which apparently you either didn't want or couldn't understand. That site provided the format and what you need to fill in the form - other than that, without the STATE no one can help you.

    I answered on another thread - WHAT STATE (for the third time).

    I posted the form for one State, the one that was the most quick to find.

    It's QDRO - and the form IS available from the administrator of the "fund" if all else fails.

Its really unfair to mark a note as unhelpful if the person is asking for more info.

    As to where you get the forms. The site I linked to as instructions for draftng a QDRO. It is not as simple as filling out a form. Don't you have a divorce attorney handling your divorce? They should be doing this for you.
