What is the procedure for taking a child out of state with a court order
I want to move to NC from Ohio. I am the residential parent but the father has shared custody with me, the 14 year old daughter's mother. What is the legal procedure to take the child with me?

If there is already a court ordered visitation in place then you must approach the court again, if you want to make a change, and convince the court that the changes you want are in the best interests of the child involved. Both parents must agree to the change.

Also when making your plans for a move away be prepared to pay the full brunt of expenses involved for visitation. So depending on the arrangement it can be very expensive.
