I am a immigrant mother of a child who was born in NC. I AM THE ONLY PARENT ON THE CHILD'S BIRTH CERTIFICATE. THERE IS NO FATHER ON IT! I need to know how can I change child's last name on the birth certificate to my new last name? WE currently live in state of NY. I was able to get a US issued passport for the child but now I wish to change or add my new last name to the birth certificate. Please tell me if this is possible and if so how can I go about this?

Normally it will involve finding and getting the fathers permission also. Or a court order to allow it without father.

Since there is no father on birth certificate, whose last name, yours are the father is on birth certificate, and to whose name do you wish to change it to
