"Cutting" is called an episiotomy. Stretching, especially with the first child, is not always possible. The vaginal canal does stretch but the outside, perineal area does not always.
Anyway, back to the subject at hand, it is the peroneal (sp) area in both sexes. Believe me, I have done enough peroneal care this past fall, on both men and women, to last me a lifetime.
It should not rip if you have a good doctor. That is what the episiotomy is for.
