1986 toyota truck 4x4, 4cyl, 22RE (fuel injection)
I have replaced all the hoses, tested everything that can be tested with meter, replaced fuel pump, timing belt, O2 sensor, a few sensors, dist. cap, rotor button, spark plugs, wires, AFM, fuel filter(several times), cleaned EGR, EGR modulator filter, injectors, cleaned throttle body. Went through all connectors.
Dropped fuel tank and cleaned it. Replaced hose from AFM to TB. Checked spark, good. Check compression, all 165 +- 5. Pulled spark plug wires while running first 3 no change and fourth killed engine. Set timing.
I lifted the rear end off the ground to check if the wheels are locked up, they moved freely.

my Dad bought the truck 4 years ago, it never ran right. First it wouldn't idle, to low, now it idles fine.
In 2wd, it barely moves, wants to die, in 4H it goes a little better and in 4L it doesn't move at all.
What am I missing??

In most of these situations, the problem is with the ignition system. Have you ever replaced the ignition coil and condenser? The coil may be breaking down under load. Did you install the same NGK spark plugs that originally came in the truck?

I do have NGK spark plugs.
Have tested the coil and reads like specs call for.
I will replace both and see how that goes.

When testing for spark, you want to ground a plug. Otherwise, you can over stress and damage the coil. Bench testing coils tends to be pretty useless, since it does not simulate conditions under load. I hope it solves your problem.

I'm assuming the problem is not with the transmission.

I see you lifted the rear wheels to see if they spin, what if a front wheel is locked up? Just a thought.
