Consequences of not changing child's last name on birth certificate
I was ordered to change child's last name on birth certificate after paternity was established. I have been collecting child support from father. Could I be in trouble for not following court order?
    Thanks

Any time you don't follow the exact directions a court orders you to do, the result is contempt of court. So yes you could get in trouble.

First, since this is a question of law, its been moved from the Children forum to the appropriate forum.

I'm kind of shocked you have to ask this question. A court order is a legal requirement. Defying ANY order of the court can result in a contempt of court citation which could mean fines and/or imprisonment.

You comply with the court order. Take a copy of the order to the bureau that issues birth certificates and ask that the birth certificates be corrected and reissued.

The next issue that I have is that my child's father was ordered to maintain health insurance for my child under the name that was supposed to be on the birth certificate. What should I do?

Not sure I follow the question. Has he provided such insurance? If so, was it in the name you were supposed to change it to?

So change the name. This is now another consequence of not following the court order. The insurance company may not pay your claims since the name is different. So stop playing games and find out where to go and go today.
