    Received form 5498, non resident alien, how and where to report?
    Hi,
    My visa status
    F1-OPT - Jan 2009-Sep-2009
    H1b - since Oct-2009

    I have received form 5498 for SIMPLE IRA contributions. Where on 1040NR or 1040NREZ shall I be reporting it?

    You do NOT report it if the contributions was pre-tax contributions from your employer.

    If they were YOUR contributions, then they need to be reported as IRA contributions on Page 1 of Form 1040NR.
