1. Yes, using the A/C increases gas consumption; blower speed has no bearing.
2. Feel the heat radiating from the elements. Use multimeter to test for voltage between the positive terminal and body ground, with the ignition switch and the defogger switch ON. There should be battery voltage--if not, check all under hood and under dash fuses with the multimeter. Check for continuity between the negative terminal and body ground. If there's no continuity, check for an open in the defogger ground wire. Touch the voltmeter positive probe to the middle of each defogger wire, and the negative. Probe to the negative terminal. There should be approximately 6 volts with the ignition switch and the defogger switch ON. If the voltage is 6 volts, the defogger wire is OK. If there is battery voltage, the defogger wire is broken on the negative side. If there is no voltage, the defogger wire is broken on the positive side. It's a good idea to clean connectors with abrasive cloth and apply thin a coating of dielectric grease, since corrosion can be a problem.
3. Car bounce, stability, handling, and tire wear. No periodic maintenance required.

5. Turn the A/C on, set to recirculate, to prevent fogging at any time. It's counter-intuitive, but turn the A/C on, set to recirculate, at the same time the heater is on in the winter.
