    Writing an Insurance Waiver
    I run an organization that works with children and teens. We require a medical information form and copy of health insurance cards for our records. One of our teens does not have medical insurance, and I was told by my bosses to write a document that states that they do not have medical insurance and that they will not hold our organization liable for any medical costs. They would then need to have it notarized.

    How do I write this so that it sounds professional? If anyone has a sample of what they might use, I would really appreciate it! Thanks!

    What state is this in ? Many states now have insurance that covers children under age 18. Also is this for sporting type events ? Aren't you required to carry field insurance should something happen while the child participates ?

    It is not for sports. It is a scholarship pageant. I just need a letter that states that we are not responsible for any medical payments.
