Birth Certificate
I was wondering what forms are needed in order for me add my name to two of my sons birth certificate?

Depends on the situation. Why were you left off them and what is the status of your current relationship to the mother and the kids? (I know you are their father... I mean do you have custody, are you all together... what's the deal)

A birth certificate is a legal document. Generally a court has to order it changed.
Knowing the circumstances would help us help you.

My boys are both of age, I got a copy of their birth certificates, and my name is not on it as the father but they are my sons, and they both have my last name.

I don't want you to get me wrong and I do appreciate you responding, but you did not really respond. My sons are both of age, I just need to have my name on their birth certificates so should something happen to me they are not left out in the cold. My boys both have my last name, am not real sure as to why I am not on the birth certificate.

Of age - as in over 18?
Where is their mother in all of this and why were you not on the B/C in the first place?
Basically what has to happen is you go to the local family court and file for a modification to the certificate. Its usually a pretty simple procedure and each state will have different forms to fill out. You may have to pay a small fee - 20 or 30. But the place to start is your local family/civil court.

I agree what does "of age" mean. Sometimes, a person's first post does not give us enough information to give a good answer. In those cases, we need to ask for more information. There is nothing wrong in that.
To give you an accurate answer we need to know the info asked for. We need to understand why you weren't on the because in the first place (apparently for 2 children) and what are the current circumstances. We are not saying they aren't your kids, but there are different ways this may need to be handled.
