    1099 w-2 box 12
    Box 12 ,state distribution, is blank. I don't have an amount to put in.

    Please clarify - are you talking about form 1099-MISC, or W-2?

    Box 12 on your W-2 is where your employer indicates a reason code and amount for contributions you made to a 401(k) or other salary deferral plans, or repors other tax issues such as group life insurance he provided in excess of $50K. If this box applies to you there should be a reason code letter uch as a "D" or "E" and an amount listed. If Box 12 is blank that means that you had no salary deferrals or other issues. That's perfectly OK, unless you think there's an error. What amount did you expect to be reported in Box 12?
