    Hi, kwiwi!

    Greetings and WELCOME to the site!     I would assume you want the unemployment office to send you a W2 form to show the income you received from them??

    Most will mail out on or by Jan 31, if you have not received it within about a week after that, contact them.

    Many have them online, if you were filing your weekly certification online, just go back to your log on and see if they have it available.

    Unemployment compensation is normally handled by the state Department of Labor.

    That same department issued the Form 1099-G (not W-2) to documen the payment of unemployment compensation for income tax purposes.

    Go to the state website and look in the Department of Labor part of the website. You will probably find that you can rpint off the Form 1099-G from the website.
