Can I use the 1040ez for my 401k ?
What form do I use for 401k?
You use NO form for 401K.
It is documented on the W-2 and is NOT reported on your annual rax return.

Ebaines answer is accurate. If the 401K issue is one of a distribution, then you must report the distribution on Form 1040.
