How do I safely wire a jetted bathtub with a heater to an old ungrounded 2- wire system?

You really can't...
The electrical code, 680.42(C) requires a grounding conductor to be run with the wiring. You can't add it separately. There are other requirements in this code section you should also make sure are included.
