Two suggestions:

1. If you kept any of the old statements from this 401(k), they should have the account number and contact information.
2. If not, contact the HR department at your old employer, and find out who the administrator is and how to contact them.
3. If that doesn't work, and you can remember who the administrator was (i.e, Fidelity, Vanguard, etc), you should be able to find a customer service contact number on their web site.
4. Failing all that, I believe that all states now maintain on-line tools for searching for abandoned property. If you haven't received any statements in 15 years, it may be that the investment firm has turned the assets over to the state as abandoned property (as required by law). Do a Google search and you should be able to find a web site that lets you search for abandoned property in your state.

Since a 401K is a retirement account your Social Security number is attached to it. Your first step is to contact the employer the plan was with. If that fails, you can go to the SSA and ask them to research and retirement plans for your SSN.
