He needs to go to the court that issued the papers and ask what needs to be done. He may have to restart or the judge could declare a judgment by default. We really can't answer as it will be up to the specific court.

This paragraph is unclear. Your BF's mom has the papers which should have been filed? Are you saying that his wife uses both last names or his mother? In either case, it doesn't really matter as far as you or your boyfriend is concerned. What does matter is whether the wife can be located for service of process. I expect the court will require that she be served with notice that he is re-opening the divorce case.

Did a judge grant the divorce but the last sections of the paperwork were never filed ? Or was this the final papers before the divorce was to be granted?

Doubtful that a judge would sign the final judgment if there were papers missing.

Maybe not sign but can grant it without getting a signature before the judge. Then after its in writing as agreed in court it gets signed. Such as any final stipulations or other misc orders.
