I had bleeding for two weeks, and it started back up with sex. I have extreme anxiety (not normal for me), fatigue, dizziness. The cramps have stopped, as has the bleeding, but the bleeding starts with orgasm.

Since it has been in there for over three years, I assume you've talked these symptoms over with your doctor?

It might be time to investigate another for of birth control if you're getting these symptoms.
