You need to marry her mother, and get formal written permission from the bio father.
It's a long process.

Any question on law needs to include your general locale as laws vary by area. Most areas do require a couple to be married to do a partner adoption. Its usually referred to as a step parent adoption for that reason.

The father will have to be consulted and approve of the adoption. Though in some cases, the bio parent's objections can be overridden by a court.
