    You go to a doctors office... they take samples to send to a lab for analysis and you wait for an answer.

    Addendum to what Smoothy said:
    Doctor might give you a pre-emptive prescription or, once they confirm the result, they might prescribe a antibiotic. Take the antibiotic until all the pills are gone; even after the symptoms have subsided.

    After antibiotics prescription is done take probiotics to replace good bacteria.

    You go to the doctor's office, you pee in a cup, and the doctor's office looks at your urine under a microscope as well as a litmus paper type test. The urine is also run on a specific laboratory machinery.

    Most of the time the urine is not sent out, and is done in the office in a matter of minutes.

    Urine samples, and normally they test your urine there but if they want to know what caused the UTI, they send a sample of the urine to the lab for a culture.

    And as was mentioned... labs can be in house or out of house. Depending on the size of the medical practice.
