Why do you need to change the name? What country are you located in?

Generally a name change requires a court order and both parents agreement. Without knowing more details about your situation like general area, and the reasons for the change, its hard to help.

In general, both parents will sign and agree to name change and a court order will be issues.

But you have not supplied enough info for this
