What constitutes a California IRA disbursement? Does it have to be from a CA bank?
On the CA 540A income tax form, Line 14g is where one is supposed to list any California IRA Disbursements. What exactly constitutes a CA IRA disbursement?

ANY IRA disbursement to a resident of California is a California IRA disbursement.

Actually it's possible that the amount of the "California IRA distribution" could be different than the actual amount of the distribution as reported on your federal return. If you made tax-deductible contributions to your IRA while a CA resident in the year 1975 and/or years 1982 through 1986, the amount you could deduct on your CA taxes in those years was less than the amount you could deduct on your federal taxes. Consequently when you take a distribution the CA taxable amount of that distribution is less than the taxable amount as reported on your federal taxes. Line 14e of form 540A is where you report this difference. But again it only applies if you were a CA resident in those years and made IRA contributions then as well. See the instructions for form 540A for details:
