I'm 4weeks pregnant found out last Tuesday. Last Thursday I started cramping heavily this continued into Saturday when I started spotting. Early Sunday morning I passed a sac like thing full of blood clots and have been spotting and cramping since. My question is could I have miscarried?

Yes, perhaps. I obviously can't tell with out an examination or blood work.

But, based on your description of the clot I think you may want to see a doctor as soon as you can.
