This has six outlet, you plug it into a existing outlet. Only 5 outlets work.

But is it the Leviton that should be replaced. Or is there something wrong

I am not sure, but if 5 outlets work, and one is not working it is the
Converter??

If the outlet was bad, none would work. Get another converter, they are inexpensive.

Your outlet is working if you get power to any part of the Leviton, its more likely an adapter than a converter. I agree, they are only about $15 and I would not trust it if one of the six receptacles is bad. There is an open circuit which could become a short.
