My niece is coming from Russia to live with us and will have to enroll into 8th grade. Our plans for her to graduate High School and go to college here. She is a US citizen. I will need to add her to my Medical Insurance if that's possible, since she is not my daughter.

Yes, you need to become her legal guardian. A letter from her parents appointing you as such should be sufficient for you to go to court to make it legal.
