    I have a 4 month old baby and I want his last name change how do I get change

    Where? What are you changing it from and to? Father's last name to yours? Something else?

    Are you married or single?

    Is the father the "acknowledged" father - he has agreed that he is the father or the Court has tested him and says he is - ?
