Had be been drinking? Was there enough foreplay? Does he masturbate a lot? Is this the first time this has happened? Does he use drugs? How old is he?

Then it's most likely not a big issue. If it happens more often, becomes an issue every time you have sex, then he should see a doctor.

It really sounds like he just wasn't aroused enough to maintain an erection. It happens.

Bottom line, if he's concerned he'll have to see a doctor. If this has only happened twice in his lifetime, I wouldn't be too worried, but if he is, the only way to find out what's going on, if there is a medical issue, is to see his doctor. We can't diagnose online, we can only guess, and my guess would be that he just wasn't that aroused at that moment, or he was high, or he had recently masturbated and it was too soon for him to go another round, he was tired, drunk, or just not that into having sex right then. All guesses, but that's all we can do.

Tell him to see a doctor if he's worried.

At times guys just lose the erection, sometimes they can't get one, that is how men work.
Stress worry, something else on your mind, the flu, other illness all can cause it
