Cat can't be pregnant, there is no uterus so it would not matter if the cat (and this is unlikely) had a third ovary.

I seriously doubt it.

Your title says that the cat was spayed twice. How is that possible? Once you remove the uterus and the ovaries, that's it. There's no need to do it a second time. They don't grow back.

Are you sure you're asking about spaying? Did you mean something else?
