I live in Wisconsin but I was born in Chicago, IL. I am 26 years old. My fathers name is not listed on my birth certificate. I need it added to obtain a Visa for school. What can I do to get his name added to my birth certificate?

Did your father ever prove paternity though DNA test ? Was he court ordered to pay child support. What proof that a certain man is the father ? Is he willing to sign to have his name added ?
