You should never get your cartilage pierced with a gun, it could shatter the cartilage. Always have it done with a needle.

As for pain, I haven't had it done, but my friend, who has many piercings, said that the cartilage was the most painful. Even then, he said it wasn't terrible, just hurt more than other piercings.
