    By tissue holder I assume you mean this

    I also assume that it is plastic and floats. If it is not visible in the bottom of the bowl, it has probably passed the trap and is in the main sewer line somewhere. Not much you can do but hope that it makes it to the street.

    If you begin to have sewer backups you will have to snake the line. For now just wait and hope. You will know in a day or two.

    Tough one, most likely it is caught within the toilet. If you lift it up, and look from the bottom, you will see up into the "curve" section and will most likely see it.

    Getting it out is a bigger issue, small hole, and no way to get a good hold on it.

    The one I got in mine before, had the spring and it expanded full size inside toilet.

    We took out toilet out into the yard, could see the holder, We ended up breaking the toilet before it ever came out.

    If this were my call the first thing I'd do is auger the toilet bowl (see image) and attempt to either retrieve it or break it up and send it out to the main. Good luck, Tom
