How do you replace a male plug/cover on an existing female cleanout that is not a perfect round anymore?

It would help me to answer your question if I knew the material the pipe and cleanout are made of, OK?
Are we talking cast iron, galvanized metal, or PVC/ABS plastic pipe here?
I can tell you that if you have metal pipes and are trying to install a cleanout then one option besides cutting-out/removing the old cleanout and installing a new one is to TRY to find a properly sized universal LEAD CLEANOUT PLUG (see image). Here, the lead cleanout plug (only available online or at a local plumbing wholesaler) is slightly malleable and you can CUT threads into them as you turn the plug into the cleanout... works pretty good.
Another option might be to purchase a wing-nut type rubber plug... see image.

Thanks for the prompt answer.
The clean out is PVC. It is 6" in diameter (female) and it is located in a concrete driveway (about an inch above the driveway). I bought a male plug but could not screw it because the existing female clean out is not perfectly round.
