Sand coming out of one floor drain in basment

Please elaborate a little here for us, OK? How is sand coming up? When fixtures are used? If so, when what fixtures are used? How about volume of water with the sand... a little or a lot? How about when you use the washing machine.. any "overflow" at the floor drain then?
