Start -> Run -> Cmd -> Netstat -a

You can also use a firewall which has a "Net State" function that can show the currently active connections and the details of these connections, including Remote Address/Port, Local Application/Port, Protocol and Status.
I know Armor2net Personal Firewall has that feature. You can have a try.
