Can you tell me what happens when nobody has insurance but there was a bad accident at the fault of the other driver? He was sited with negligent driving etc, but huge medical bills and a truck totaled.

If neither of you have insurance, and you believe it was the other driver's fault, it is up to you to sue the other driver.
