My mother thought she added her last name to my birth records and come to find out it was my fathers last name on my records. How can I add my mothers last name to birth certificate and social security card? P.S she does not no how to read or write. She could not understand how to add it to the birth paperwork.

How old are you ?

If you are over 18, and in the US, you just go to the court house and do a legal name change, this allows you to change your drivers license, and SS card and so on.

IF you are under 18, then you will have to get your fathers permission for this also
