I had a child 14 years ago and didn't put his fathers name on the birth certificate as we had separated.. Now we are reconciling and both my husband and I with our child all agree to having his dads name put on the birth certificate. But we don't how, any help would be appreciated.. I am in Melbourne, Australia.
