My daughter doesn't have my last name or her fathers. How do I change her last name to mines?

It varies by State and the age of the child but it normally takes a Court Order. You request the name change, give the reasons, the Court either orders the change or doesn't.

I live in pa. She has my ex girlfriends last name. She was extremely abusive towards me n when I had my daughter she threatened my life if I didn't give my daughter her last name.

Doesn't matter. A birth certificate is a legal document. To change it requires a court order. Such a court order will need the approval of both parents.

She only has one parent.. ME I'm the only name on her birth certificate. Her birth father left the country the week after I told him I was pregnant.

Sorry, but unless you had artificial insemination the child has 2 parents. Whether you listed the other parent on the birth certificate or not does not change this biologicial fact.

Therefore a court will require that you identify the father and, at least, make a good faith attempt to contact him and have him agree to the name change.

No, unless this is some sort of virgin birth, your daughter has two parents.

So I have this right - you were having sex with a man but your girlfriend was a female. Lots of people are bisexual. I know - your girlfriend theatened you and so -

At any rate, you need a Court Order to change the last name.
