My 12 week old puppy is sick what do I do

What you do is call a vet! No one should have a pet without a vet.
You haven't even said HOW he's sick, whether he's had his shots, how long you've had him, so CALL A VET!

I've had him 12 weeks and yes he's had his shots vomiting diarrhea weight loss

Will you take him to a vet? He may have a very bad disease that puppies get -- parvo.
