That could mean anything. For instance, it could mean you had your tonsils out and were in a horrific house fire.

At 40 you are reaching the age where pregnancy would be considered high risk with a high risk of deficits to the fetus.

Since you appear to have had a tubal ligation, it is possible that it be reversed, but pregnancy is never a guarantee due to scar tissue on the fallopian tubes, and tubal pregnancy would be increased.
