What is the statue of limitations on billing a patient?
How long in the state of Utah do we legally have the right to bill a patient for their claims not paid or coinsurance after payment has been applied from insurance?

It's Statute, not statue. You can bill forever. You can only take legal action for: Oral contracts, 4 years; Written contracts, 6 years.

I'm assuming this was an oral contract/agreement to pay for services - obviously so, because your office was furnished with the insurance info.
