I have my scan booked in for the 31st of this month, but I have recently moved back home as my partner and I split up, so is there any way I can transfer the scan to my local hospital and how?

Just call your doctors office (or whoever arranged the scan in the first place) and ask if they can/will book it at your local hospital.
