    There is no one correct answer to this, but generally speaking, and for most residential work, it is 6'7" to the top of the actuator/handle/whatever.

    It depends on the use. Typically it is as stanfortyman said. 6' 7" to the middle of the handle when it it at it's heights point, but the code allows exceptions like if it's attached to a motor, there is no defined height so long as it is 'easily accessable'. What is your application, and why do you want to mount it so high?
