I was sentenced to 1 year in jail with time served. But when I got out I am still on probation. Is it possible that they just don't have my information right?

My Po didn't have any of my papers when I went in to see him and I went to check in again and he was on vaction so Im just wandering if this is possible. Because before I went to jail I was sentenced to 2 1/2 years probation but when I went to jail I was sentenced to 1 year with time served...
