    How to get my name on my child's birth certificate
    I'm jamaican and my child and his mother is american. The child has my last name but my name is not on the birth certificate. How do I go about getting my name affixed to it?

    Is the mom OK with it, or have you proven paternity though the courts ?

    The mom is okay with it. We just want to know the process. State of birth is New York.

    There are several ways to do it.
    1 You and the mother sign Acknowledgment of paternity.
    2.You go through Court procedures.
